Introduction

  • Arthrotomy, the usual method for synovial biopsy, allows examination of joint and removal of specimen (membrane 2-4mm wide, length dependent on size of joint) from most suitable area.

Uses

  • Diagnosis of suspected inflammatory arthropathies:
    • Degenerative joint disease (DJD).
    • Immune-mediated arthritides.
    • Rheumatoid arthritis.
  • Diagnosis of joint infections.
  • Confirmation of diagnosis of synovial neoplasia:
  • Specimen may be used for histopathology, microbiological culture or immunohistopathologic examinations.

Technical problems

  • Synovial membrane histopathology - overlap between DJD and immune-mediated arthropathy requires cautious interpretation.

Alternative techniques

  • Synovial fluid aspiration.
