Overview

  • Comprises 5 isoenzymes not organ specific, but useful in assessment of elevations in LDH in which the underlying cause is not apparent:
    • LD1: heart muscle, brain, testes.
    • LD2: smooth muscle, heart muscle, brain, kidney, bone and thymus.
    • LD3: all major organs and smooth muscle.
    • LD4: intestine, liver and skin.
    • LD5: skeletal muscle, skin, liver and intestine. Isosenzyme modestly elevated with liver disease   Blood biochemistry: lactate dehydrogenase - normal isoenzyme analysis   and markedly raised with skeletal myopathy.
  • Longer half-life than creatinine phosphokinase (CK or CPK)   Blood: biochemistry - creatine kinase  .

Uses

In combination

  • With total LDH, LDH isoenzymes and/or other test results for evaluation of muscle damage or liver disease.

Methodologies

  • Enzymatic method varies between laboratories (electrophoresis needed for LDH isoenymes).

Availability

  • Limited availability.

Technique intrinsic limitations

  • Test results most useful when interpreted in conjunction with other laboratory results and clinical findings.
LDH without isoenzymes is so non-specific that it is rarely diagnostically useful.
