Tarsus: desmitis - plantar ligament

Introduction

  • Cause: biomechanical stress due to poor conformation   Musculoskeletal: conformation  , intense exercise, external trauma.
  • Signs: enlarged plantar surface of the fibular tarsal bone   Plantar ligament: desmitis  , acute - heat, pain, lameness, elevated heel, cellulitis.
  • Diagnosis: physical examination   Musculoskeletal: physical examination - adult  , radiography   Hindlimb: radiography  , ultrasonography .
  • Treatment: rest, corticosteroid injection   Dexamethasone  , as long as curb due to low grade desmitis or subcutaneous hematoma and not due to soft injury or moderate fiber tearing of plantar ligament.
  • Prognosis: good if no conformational faults.

Pathogenesis

Etiology

  • Trauma.

Pathophysiology

  • Faulty conformation   Musculoskeletal: conformation  (sickle   Hindlimb: sickle hocks 01  and cow hocks) for foot/shoeing angle.   →   chronic abnormal biomechanical stresses   →   cyclical fatigue of ligament   →   repeated injury and repair phases   →   inflammation, scarring, remodeling.
  • Direct trauma (kicks, violent hock extension)   →   severe inflammation   →   damaged collagen replaced with Type III collagen (scar tissue).
