Introduction
- Approach to the renal collecting system via the parenchyma.
Uses
- Removal of uroliths Urolithiasis.
- Exploration of the renal pelvis for investigation of neoplasia Kidney: neoplasia or hematuria (rare) Hematuria.
Advantages
Disadvantages
- Damage to renal parenchyma.
- Renal function impaired (up to 20-40%) for short period.
Alternative techniques
- Medical management of renal uroliths unless renal function deteriorating, uroliths not amenable to dissolution, or presence of severe pain or infection.
Decision taking
Criteria for choosing test
Renal function- If deteriorating cannot rely on medical management.
Risk assessment
Bilateral procedures- May be staggered since renal function is impaired post-operatively for short period (up to 20-40%) Anesthesia: in renal insufficiency.
