Tarsus: plantar instability

Synonym(s): Proximal intertarsal subluxation, Plantar tarsal ligament rupture


Introduction

  • Calcaneoquartal subluxation due to plantar-tarsal ligament rupture.
  • Common injury in racing/coursing dogs and middle-aged Collie types, especially Shetland Sheepdog; bilateral involvement not uncommon.
  • Cause: in the athlete usually occurs during racing, usually isolated injury but may be in association with other tarsal fractures/luxation, especially centrodistal luxation.
  • In domestic pet (Collie, Shetland Sheepdog) rarely present with history of trauma - may not be presented until problem is bilateral.
  • Signs: joint pain, plantigrade stance with proximal intertarsal hyperextension.
  • Diagnosis: examination.
  • Treatment: calcaneoquartal arthrodesis.
  • Prognosis: good if adequate treatment.

Etiology

  • Athlete - stresses of racing.
  • Pet - rarely a history of trauma, ligament degeneration?
