Stomach: lymphocytic plasmacytic gastritis

Introduction

  • A chronic inflammatory disease of the stomach.
  • Cause: rarely determined but may include dietary hypersensitivity or intolerance.
  • Concurrent intestinal involvement may occur.
  • Diagnosis: biopsy findings; exclusion of other causes of gastric inflammation.
  • Treatment: hypoallergenic diet, prednisolone, anti-emetics; azathioprine in severe cases.
  • Prognosis: generally good.

Pathogenesis

Etiology

  • Majority of cases are described as idiopathic.
  • Dietary hypersensitivity Skin: food hypersensitivity /food intolerance is thought to be important in some cases.
  • Some cases have concurrentHelicobacterinfection Helicobacter spp - significance not known.

Predisposing factors

General
  • None identified.

Pathophysiology

  • An inflammatory reaction to an unknown stimulas results in the accumulation of lymphocytes and plasma cells within the lamina propria.
  • Gastric lymphosarcoma Lymphoma may appear similar to lymphocytic plasmacytic gastritis in its early stages.

Timecourse

  • Months.
