Introduction

  • Cause:
    • Alkalis'melt' the cornea and readily penetrate the eye Cornea ulcer 03.
    • Acidscoagulate proteins and penetration of the eye is limited Cornea ulcer 03.
    • Detergentsandalcohol, eg surgical spirit strip corneal epithelium so care must be taken when preparing the eye for surgery.
  • Treatment: flush with copious quantities of physiological saline.
  • Prognosis: alkali burns - poor; prolonged irrigation with saline and use of a collagenase inhibitor recommended. Reconstitute 2 ml EDTA blood collection tube with sterile water or serum and use for frequent irrigation. Hospitalization required.

Pathophysiology

  • Alkaline corneal injury causes profound collagenolysis without coagulation limiting the extent of chemical penetration.

Timecourse

  • Acute - collagenolysis.
  • Chronic - corneal scarring.
