Pathophysiology

  • Bone and cartilage fragment from distal aspect (apex) of patella.
  • Changes in biomechanics induced by medial patellar desmotomy   →    change in position of patella in intertrochlear groove or instability   →    fragmentation 3 weeks to 12 months post-operatively.
  • Following repetitive trauma to the patella region, eg upward fixation of the patella    →    chronic changes in the biomechanical forces on the patella.
  • The incidence of this condition has fallen now that medial patellar desmotomy is less commonly performed.
