Medial patellar ligament: splitting
Introduction
- Induces a localized desmitis within the ligament with subsequent ligament thickening; this disables the proximal part of the ligament from hooking easily over the medial femoral trochlear ridge.
Uses
- Treatment of upward fixation of the patella Patella: upward fixation which has not responded to conservative measures.
- Alternative to medial patellar desmotomy Medial patellar ligament: desmotomy in which the medial patellar ligament is transected as a surgical treatment.
Advantages
- Can be carried out in standing position if desired.
- Simple technique.
- No known complications.
- High success rate.
- Rapid return to exercise.
Disadvantages
- Rarely, a case may not respond initially, or in the longer term.
- The technique can be repeated with some success.
Technical problems
- In very small animals, it may be difficult to visualize and reach the surgical sites.
- In these cases, and those where behavior issues are a problem, a short general anesthetic and dorsal recumbency may be required.

Preparation
- Standing sedation Anesthesia: standing chemical restraint and aseptic preparation: 10-15 min.
- Induction of general anesthesia Anesthesia: induction - overview and aseptic preparation: 20-30 min.
Procedure
- Surgical technique: 10-15 min.

- Persistent and unresponsive upward fixation of the patella Patella: upward fixation .

- Minimal.
