Stomach: lymphocytic plasmacytic gastritis
Introduction
- A chronic inflammatory disease of the stomach.
- Cause: rarely determined but may include dietary hypersensitivity or intolerance.
- Concurrent intestinal involvement may occur.
- Diagnosis: biopsy findings; exclusion of other causes of gastric inflammation.
- Treatment: hypoallergenic diet, prednisolone, anti-emetics; azathioprine in severe cases.
- Prognosis: generally good.
Presenting signs
- Chronic vomiting.
- Weight loss.
- Diarrhea.
Cost considerations
- Investigation may be prolonged.
- Diagnosis requires biopsy and long-term medication may be required.
Pathogenesis
Etiology
- Majority of cases are described as idiopathic.
- Dietary hypersensitivity Skin: food hypersensitivity /food intolerance is thought to be important in some cases.
- Some cases have concurrentHelicobacterinfection Helicobacter spp - significance not known.
Predisposing factors
General- None identified.
Pathophysiology
- An inflammatory reaction to an unknown stimulas results in the accumulation of lymphocytes and plasma cells within the lamina propria.
- Gastric lymphosarcoma Lymphoma may appear similar to lymphocytic plasmacytic gastritis in its early stages.
