Introduction

  • See pyloromyotomy Pyloromyotomy for all details except technique.

Uses

  • Congenital pyloric stenosis.
  • Chronic hypertrophic pyloric gastropathy (CHPG) (some say Bilroth 1 better in these cases).
  • Removal of some pyloric polyps (Y-U).

Advantages

  • Allows full thickness pyloric biopsy to be taken.
  • Allows excision of hypertrophied mucosa.

Disadvantages

  • Increased risk of contamination.
  • Heineke-Mikulicz may distort pyloric anatomy if significant thickening is present.
  • More extensive procedure than pyloromyotomy.
