Introduction
- Intractable/recurrent diarrhea of small or large bowel origin.
- Cause:
- Dietary intolerance Skin: food hypersensitivity.
- Parasites.
- Infection.
- Maldigestion Maldigestion.
- Malabsorption Malabsorption.
- Treatment: depends on etiology.
- Prognosis: guarded.
Presenting signs
- Variable dependent on etiology.
Breed/Species predisposition
- German Shepherd dog German Shepherd Dog - small bowel.
- Basenji Basenji - small bowel.
- Irish Setter - small bowel.
- Shar Pei Chinese Shar Pei - small bowel.
- Boxer Boxer - large bowel.
- Golden Retriever Retriever: Golden - large bowel (in UK).
Pathogenesis
Etiology
- See specific conditions.
Pathophysiology
- Impaired digestion/impaired absorption Malabsorption → presence of intraluminal complex molecules → osmotic diarrhea.
- Some bacterial toxins, and also metabolites of intraluminal bacterial digestion of nutrients, stimulate secretion of fluid from intestine → secretory diarrhea.
- Intestinal mucosal inflammation → increased mucosal permeability → diarrhea.
- Altered intestinal motility.
Timecourse
- Persisting >2 weeks.
