Skin: subcorneal pustular dermatosis

Introduction

  • Rare if differential diagnoses carefully ruled out.
  • Cause: unknown, but probably immune-mediated.
  • Breed: 40% of cases in miniature Schnauzer.
  • Diagnosis: no definitve test; rule out differentials.
  • Treatment: dapsone most effective.
  • Prognosis: good if responds to treatment.

Presenting signs

  • Pustules.
  • Head and trunk affected.
  • Pustules non-follicular.
  • Epidermal collarettes.
  • Crusting, scaling, erosion.
  • Foot pads may be involved.

Age predisposition

  • Any age from 6 months may be affected.

Breed/Species predisposition

Cost considerations

  • Condition may require long-term treatment or be recurrent.

Pathogenesis

Etiology

  • Not known, but likely to be immune-mediated.

Pathophysiology

  • Immune complexes and IgA in stratum corneum → neutrophil chemotaxis and subcorneal pustule formation.
  • IgA and immune complexes have been demonstrated in the skin which may lead to neutrophil chemotaxis and subsequent subcorneal pustule formation.

Timecourse

  • Pustules appear rapidly and are transient.
  • Disease is chronic unless treated.
