Skin: discoid lupus erythematosus

Synonym(s): Cutaneous lupus erythematosus

Introduction

  • Uncommon auto-immune skin disease principally affecting the nose.
  • Cause: unknown.
  • Signs: nasal depigmentation, erythema and scaling.
  • Diagnosis: history, clinical signs, histopathology.
  • Treatment: chemotherapy, UV light avoidance.
  • Prognosis: usually good.

Presenting signs

  • Early lesions show depigmentation of the nasal planum and smoothing of nasal planum, ie loss of normal 'cobblestone' appearance .
  • Later lesion - erythema and scaling Discoid lupus erythematosus secondary pustular pyoderma .
  • Subsequently there is erosion and ulceration.
  • Lips, eyelids, inner surfaces of pinnae, peri-orbital areas and distal limbs may be affected.
  • There is no systemic involvement.

Geographic incidence

  • It is probably more common in sunnier climates.

Breed/Species predisposition

Pathogenesis

Etiology

  • Unknown, but is immune-mediated.
  • A benign variant of systemic lupus erythematosus Systemic lupus erythematosus.
  • Exposure to sunlight worsens condition, so photosensitivity plays a part.

Specific

  • Sunlight worsens disease.

Timecourse

  • Slowly progressive (but tends to be limited to head and feet).
